Invasive dental procedures linked to heart inflammation

An interesting study titled “Endocarditis, invasive dental procedures, and antibiotic prophylaxis efficacy in US Medicaid patients” appears in Oral Diseases on April 27, 2023, written by Martin Thornhill and et. al. The study investigates the association between invasive dental procedures and endocarditis, which is inflammation of the inside lining of the heart chambers and heart valves. In the study the authors used the MarketScan multi-state Medicaid database to examine 1.68 million Medicaid patients with linked medical, dental, and prescription data and compared it to 7.95 million individuals with employer-provided Commercial/Medicare-Supplemental health coverage with linked dental and prescription benefits. The authors found that the cohort study showed increased endocarditis incidence within 30 days of invasive dental procedures in those at high risk, particularly after extractions or oral surgery. Oral Surgeons License Suspended for Five Years After Patients Contract Bacterial Infection of the Heart

An oral surgeon in New Jersey has agreed to a five year suspension of his license after 15 patients contracted the bacterial heart infection endocarditis at his office. The oral surgeon also agreed to pay $243,500 in penalties and $50,000 in costs totaling $293,500, to resolve the State of New Jersey’s allegations that he exposed his patients to the risk of contracting endocarditis due to his failure to follow proper infection control procedures at this office. Twelve of the fifteen patients who contracted endocarditis required subsequent surgery and one died as a result of the bacterial heart infection. Dental Plaque May Trigger Blood Clots

Oral bacteria that escape into the bloodstream are able to cause blood clots and trigger life-threatening endocarditis. Streptococcus gordonii is a normal inhabitant of the mouth and contributes to plaque that forms on the surface of teeth. However, if these bacteria enter into the blood stream through bleeding gums they can start to wreak havoc by masquerading as human proteins. Researchers from the University of Bristol and the Royal College of Surgeons in Ireland (RCSI) have discovered that S. gordonii is able to produce a molecule on its surface that lets it mimic the human protein fibrinogen — a blood-clotting factor. This activates the platelets which then clump inside blood vessels.
